Adding remarks

I am using Medisoft Advanced v. 9 sp 1. I need to send the following information on a UB in the electronic equivalent of the "Remarks".

OT
OTHER
CHANGE IN VALUE CODE 14

I need it to be on three separate lines. When I add this text in the "EDI Note" area within the claim, I can only add it all as one line. When I tried to add it in the "EDI Notes" within the case, it doesn't get sent at all.

You should use the UB-04 Smart Screen which will add the UB-04 tab in the Case. On that tab, you will see 4 fields for up to 4 remarks lines.

The data that I put in the Remarks section of the UB Smart Screen is not included/transmitted on the electronic claim.

So it looks like for electronic claims the claim note comes from the Claim -> EDI Note tab -> Claim Note Frame. Enter the code (qualifier code) and text for the note.

This defaults from the Case -> Comment tab -> EDI Notes Frame.

It look like the Remarks on the UB-04 Smart Screen aren't used anymore. I'll ask around.

Yes, but putting the EDI notes in the claim doesn't allow me to split the remarks into 3 separate lines.

You are correct. There is an 80 character limit on the single line. Are you getting a claim rejection? I hear eMDs is in the process fixing and improving Revenue Management. It may be too late to get anything into this round of changes, but if you can present me with a good case then maybe I can take it to eMDs and they'll get it into a future update.

For some types of claim adjustments, Medicare requires very specific data be transmitted on specific remark lines. My adjustments are not being processed because of the issue of not being able to submit the data in the format required.

Please fax me details. EOB, remark that you entered, print out of the claim, etc. There are no line numbers in the electronic file, just 80 characters of text for each type of note. Are you sure you can't just type it all into one field? Maybe it will make more sense when I see the remarks you are trying to enter.

Here's the Medicare documentation about needing to have remarks:

"If a provider submits an adjustment with condition code D9, and there are no remarks, or they do not have any of the remarks (on the second line of remarks) listed below "verbatim," then Medicare will return the claims back to the provider to correct with reason code 37541."

Well, if you can't use condition code D0 through D8, then I think you'll have to file these on paper because the instructions would only apply to paper claims.
