Billing Injectables

I have never done dermatology billing but have a new client where I have to bill for botox injections. The provider is going to give me the CPT code, but am I correct that I enter the NDC code (000233919), the unit price (which is $15) and unit for the NDC Unit of Measurement (for 1 box) AND enter N4 in the Code ID Qualifier field?

And for Code Type is it still Procedure Charge or would it be Product Charge?

That sounds right. You would still use Procedure code for this.
