DIAGNOSIS CODE CUT OFF
when I submit claims, last two digits of first diagnosis code are cut off if they exceed 4 digits, how do I prevent that from happening. thank you

As I suspect from your previous post, you are submitting claims in print-image format.
There are 2 options here:
1. Switch to ANSI 5010 Professional claims format. There is no mapping involved and the clearinghouse will receive the entire diagnosis regardless of it's length.
2. Your clearinghouse needs to update their mapping of their print image claims with you. Send a test claim with as many diagnosis codes as you will ever use and make sure all diagnosis codes are at least 4 or more digits. Then ask them to update their mapping using that test claim.
