Old Patient Information Populates a new Patients case

When we enter a new patient into medisoft then try and enter a case for them, The System is Auto populating an old Patents information into the new patients case.
I seen this happen before but it tend to disappear, but it is doing it again.
This only happens on newly entered patients, there is no problems entering a new case on an established patients in the system
Can you help?
