For DMerc, I have to have the Facility info appear on claims. For the facility under Facility IDs, I have the Insurance Carrier listed, I have the NPI listed, I checked the box Send Facility on Claim and I have 77 as the Service Location. But still the facility info does not appear on the claim. And, yes, this facility is listed on the transaction (in the case and under Facility on each line of the claim in Transaction Entry).

What is being done wrong? Why does not the Facility Info with 77 qualifier not appear?

Try making sure the facility name is different than the practice name. If they are both the same, try making the facility name slightly different by adding or removing a space or maybe abbreviating somewhere so that there is some difference.
