ICD 9 codes missing in HCFA

Post by ravi » Fri Oct 02, 2015 7:13 am

Upgraded to Medisoft V20 from Medisoft V19 a few days ago.

In Medisoft V20, set default code set to ICD-10 with effective date of 10/1/15 for all payers.

For a date of service before 10/1/15, HCFA claim file generated using 'Claim Management', 'Print/Send' button
has no ICD codes at all, neither ICD 9 nor ICD 10. Positions which have diagnosis codes are all blank.

However, the same claim when generated as paper shows correct ICD 9 codes as expected.

I would like to find out if there is any workaround to get the generated HCFA file have ICD 9 diagnosis codes.

I am using 20.0.2.68 build of Medisoft Version 20.

Where are you looking for the ICD9 and 10 codes? Are you using a custom claim report? Can you try the built in claim 1500 02/12 Primary and see how that pulls?

The ICD9 codes are missing not in some custom claim report, but in the generated HCFA claim file itself which
gets uploaded to the clearing house.

The way the HCFA file is generated is as follows:
Open 'Claims Management' window.
Click on 'Print/Send' button.
On the window that comes up, choose the 'Electronic' radio button and click on 'OK' button.
On the next window that comes up, click on 'Create File' button.
This should dump all claims in 'Ready to Send' status into the supplied file name.

ICD 9 codes are missing in this generated file. It is not that I don't know where to look for
diagnosis codes in the generated file, the submitted HCFA file was rejected by the clearing house
because of no diagnosis code for any claim in the contained file. For the first claim inside a HCFA file,
diagnosis code start in line 37 and line 37 is blank now.

Building the same claim in paper works fine and shows ICD 9 codes.

What is broken is the generated HCFA file for electronic processing.
The only workaround I can think of is to submit everything by paper instead of
clearing house.

Is this a known problem with Medisoft V20?

I actually experienced this same issue w/ V 19 but have been hoping it's been fixed in V 20....will be interested in seeing how this is resolved. Making my switch as soon as I get my first Oct 1 charge sheets. Ali

I did not have this problem with Medisoft V19. But, I was working with V19 without
enabling ICD10 as default code set and setting any date for ICD 10 migration.

This is most likely a problem after doing ICD10 migration in Medisoft, does not
matter V19 or V20.

I wanted to undo the ICD10 migration to see if the problem goes away, but
'Tools/Services/Set ICD Version' window has
'ICD-10 Effective Date' grayed out indicating ICD 10 migration is a one way path.

Ravi is right that the mappings and set icd version needs to be done. The thing is that you are using a custom report. Try printing to screen and use the non custom standard report. If you are using a report that was not modified based off the MS19 or 20 HCFA 1500 02/12 then it will not work. You need to either use the built in report or modify it.

The print image built into Medisoft does not support ICD-10 nor the 2/12 HCFA claim form for that matter. Clearinghouses can map the old claim form, but they can't map ICD-9 to ICD-10.

We recommend switching to ANSI 5010 claims using Revenue Management which supports ICD-10.
