Fix for W/Form CMS-1500 reports (Medisoft v8-10)

There is an issue with some of the 'W/Form' versions of the CMS-1500 reports that we released for Medisoft Versions 8-10 where the transaction lines in box 24 do not print properly. We have updated the forms to correct this issue, and those that purchased these reports from us that are experiencing this issue may Contact Us for their free replacement CD.
