The facility comes from the Case->Account Tab -> Facility field. Be sure that is selected and that the Name is something slightly different than the practice name or provider name. Once you have the case updated, be sure to go into Transaction Entry and make sure the correct facility is selected on the charge line. If you change the facility on the charge, it will remove the charge from the claim and you have to rebill it.
