provider telephone # does not show on CMS-1500
The last set of claims I submitted to Emdeon were rejected because the provider's telephone # did not print on the claim form. This was the 1st time claims have been rejected because of this. I looked at the past year's claims and the tele # was not printing on old claims.
The tele # is correct in practice information and in provider information.
Please help me with this problem.
Also, what is charge for 1 call for help, and 6 mo support.

Your choices are:
1. Use the Report Designer that comes with Medisoft to create your own CMS-1500 report template that includes all the required information.
2. We wrote CMS-1500 report templates that work for Medisoft v8. We sell those for $180.
3. Upgrade to Medisoft v15, which includes CMS-1500 report templates.
Our Medisoft support rates are $200 for one month of support and $600 for six months.
