Are you getting this error at your clearinghouse? Here is the most likely reason why you are getting this and how to fix it.

We see this most frequently with those that are submitting print-image claims to their clearinghouse. The issue is there are claims in Medisoft that do not have any transactions associated or just “Comment” transactions. This is most frequently caused by changing the diagnosis code on a transaction after the claim was already generated. The transaction is automatically removed from the claim and then is available to be assigned to a new claim. (A warning message pops up explaining this in transaction entry at the time the edit is made). The problem is, if the last billable transaction is removed from the claim, Medisoft does not automatically clean up and it leaves the “empty” claim sitting there. Print image claims will continually include these “empty” claims but never mark them as sent. So basically it gets included in every batch over and over again causing an error every time a batch of claims are sent.

The recommended solution is to switch over to ANSI 5010 claims on Medisoft v17 or higher and the issue is avoided entirely.

For those of you who cannot switch to ANSI 5010 or if you need to stay on an older version of Medisoft for now, here is the way to fix this. Log into your clearinghouse and see what patient you are receiving this error on. Then go into claims management and sort by chart number to see all the Ready to Send claims for that patient. Open them one by one and go to the Transactions tab. If there are only Comments but no billable procedures listed, then that claim record can simply be deleted. Repeat the process for all the other patients that have this problem.
