We just upgraded to v16 and I'm wondering how do I pull a report showing what transactions the user (login) did in the software? Like when the user deleted a transaction, entered transactions etc...

First go to Files -> Program Options -> Audit tab
Check both Update and Delete next to Transaction.
From this point in time on, you can go to Reports -> Audit Reports -> Audit Generator to generate an audit report on the transaction table. You will need to have security setup in Medisoft to see what user did what actions.

Under the Audit tab in the Program Options, I've checked the Deleted box next to appointments as we're trying to figure out who's been deleting some appointments, but the Audit report is still only showing Updates. I've tried restarting Medisoft after saving the changes, but no luck. Is there something else needing to be done in order for that to take effect?

When you turn audit on, that means Medisoft will only audit that table from that point in time forward and after everyone has restarted Medisoft and Office Hours. So if audit on that particular table was not turned on when the update or delete action occurred to the table, then nothing is stored in the audit trail.
