We recently had an insurance company deny our claim because the patient signature on file field 12 & 13 was blank but the date was filled in. in looking all of our claims print this way. I believe it used to say "signature on file" in both of those field and now do not populate. We have the patient signature on fill box check as well as assignment of benefits yet all claims print this way when printing to paper claim. Is there something else we need to do to make this happen as I believe it used to print correctly and I am not sure when it quite doing this.

On the Insurance Carrier on the Options and Codes tab there are options for both boxes.

yes, that says signature on file for both patient and insured, but it doesn't actually print that like it should.

OH! I LIED! for some reason that providers none of the insurance had that option selected. Sorry! That was my problem.
