UB-04 Claim Creation Date

My claims have always reflected the original claim submission date as "Creation Date" on the UB-04. However, recently my claims are reflecting the date of reprinting. I need them to reflect original claim submission date for obvious reasons. I am not aware of any settings being changed. How do I fix this?

Hi Beth, are you talking about the Date Created column in the Claim Management window? What version of Medisoft are you using?

Page_____of _____ CREATION DATE TOTALS--->

It's creation date I'm concerned about. I want it to reflect the original claim date. I'm going to look at the file itself and see where it is pulling info from.

Yeah, the UB04 v19 for should be printing whatever you have in the Date Created Column of the Claim Management window. Try printing with the report named UB04 (Primary) - v19. Maybe you are using a custom report.

Hi, I am having this same issue again. Yes, I use a customized UB04 due to requirements of some insurance companies. The Primary UB04 does not work for us. It is line 23 on the UB-04 form that line is as follows:
Page_____of _____ CREATION DATE TOTALS--->
When I look at the expression on the customized form it is: Print Claim. Date Created
This seems right but what it prints is the date it is being reprinted.
It seems I was able to fix it once but now it's doing the same thing!

Try printing with the standard UB04 form that came with Medisoft and see what prints on that one.
