Creating a Label for a Lab Requisition

Good Day, I am trying to learn how to create a label for use on a lab requisition form. The label needs to include the patient name, MRN, DOB, Sex, Address, Phone number and the insurance carrier name and policy number. We have been successful adding all fields except the insurance and policy number. Does anyone know how we would go about creating this label? We are in Bermuda so HIPAA does not apply here if that matters. We are also using Medisoft SP2.

You might be able to do this using macros in a Final Draft document. What are you trying to do and what isn't working? Is this Medisoft Clinical or just Medisoft?

Hi,

We have been trying this in Design Custom Report. We are using Medisoft Version 17 SP2, Office Hours and Medisoft Reports.

Might it be easier for us to retain you to create this for us? I would like to discuss a monthly remote support contract, etc. Tomorrow is a public holiday in Bermuda so we will be closed but if you have time available Thursday or Friday to discuss this option with me I would appreciate that.

That's great. For most reports, we can do for $180. If you could mock up an example in a document and include how you want to select the data along with any groupings or totals that would be great. We can then get started. You could just write in the missing elements on your existing report if that would be easier. Just fax over to us - 417-890-6788.

These were some of the best tips for lab requisition. I will follow these tips for a better implementation of the patient delivery system.
