Notification of Surescripts Potential Patient Safety Issues

Missing character in Medication History patient directions


Surescripts recently conducted an analysis of selected Medication History fill and claims data and determined that some Medication History response messages provided by a number of pharmacy data sources may have truncated the forward slash character (“/”) in the patient directions, making the related information inaccurate.

For example, the patient directions may be reported as “TAKE 1 AND 12 TABLET BY MOUTH ONCE DAILY WITH FOOD” instead of “TAKE 1 AND 1/2 TABLET BY MOUTH ONCE DAILY WITH FOOD.

RESPONSE:
Surescripts urges customers to carefully review patients’ medication product strengths and dosages, until the issue is resolved.


Missing/Truncated Strength and Form in Medication History


Surescripts recently conducted an analysis of Medication History fill and claims data and determined that some Medication History response messages provided by a number of pharmacy benefit managers (PBMs) and pharmacy data sources may have contained information that is error-prone.

For example, instead of: “Lisinopril 10MG tablet”, it is transmitted as: “Lisinopril”.

RESPONSE:
Surescripts urges customers to carefully review all prescription Drug Descriptions until the issue is resolved.

Missing leading zero (aka, “Naked decimal”) in some Medication History response messages

Surescripts recently conducted an analysis of Medication History fill and claims data and determined that some Medication History response messages provided by a number of pharmacy benefit managers (PBMs) and pharmacy data sources may have contained information that is error-prone. This potentially error-prone data is occurring in between 0.01% and 0.03% of Medication History dispensed records provided by the data sources. This issue does not affect any electronic prescriptions routed through Surescripts.

For example, instead of: “0.5 MG CAPS”, it is transmitted as: “.5 MG CAPS”

Surescripts urges customers to carefully review patients’ medication product strengths and dosages, until the issue is resolved.
