Problem on Diagnosis Code Select Screen (Practice Partner, Medisoft Clinical, Lytec MD)

I am contacting to signal you to a significant problem that effects the subsequent products: Practice Partner, Medisoft Clinical and Lytec MD.

Problem:
Please be warned that when seeking for “hyperglycemia” on the Diagnosis Code Select screen, a PRECISE description of “Chronic hyperglycemia” is sent as one of the viable search results, but it is related beside an IMPROPER ICD-9 code of 251.2 (251.2 is the ICD-9 code for Hypoglycemia). Notice: There is not a related ICD-10 code, so, we do not expect that this specific difficultuy is usually being designated. Once Chronic hyperglycemia (with ICD-9 code 251.2) is inserted to the patient difficulty catelog and/or progress memo, the description is preceise, but it shows the IMPROPER code. This will influence Billing and Drug-Disease Interaction checking, as both of these components use the ICD code rather than the difficulty description.

Reason:
This problem is due to a content mistake that Practice Partner obtained from their diagnosis codes supplier. McKesson is expecting a fix from the supplier. The expected time frame for this fix is early May. An additional announcement will be shot out once the fix is accessible.

Advice:
Please do not choose “Chronic hyperglycemia” after doing a difficulty search and be watchful, both after inserting problems/codes throughout an encounter and prescribing suitable medicines to go alongside them. To test to see if Chronic hyperglycemia is recorded on patient difficulty catalogs, run a inquiry with the Patient Inquiry report. Access: Reports > Patient Inquiry > select ‘Problem’ (option #39 under the Problem List header) for the Selection Criteria > insert the difficulty term “chronic hyperglycemia”, highlight the Active checkbox and tick OK. Tick OK on the Operator dialog. Tick OK on the Select Provider dialog. Run the report. A dialog will be presented indicating no matches have been found. No further action is needed. If you have matches, a dialog will be accessible if there are patients that agree with the range conditions. Tick OK on the dialog and print the report. Check the patients on the report and manually revise the patient difficulty catalog(s) to substitute Chronic hyperglycemia (with ICD-9 code 251.2) with an acceptable problem/code. Likewise, check any custom .MP/.DX/.OP quicktext and custom note templates to safeguard that Chronic hyperglycemia with ICD-9 code 251.2 does not happen. It is not present in any of the McKesson provided quicktext or note templates.
