Multiple Provider Practice

We are transitioning to a "medical group" and I am unclear on how to bill different providers for services on the same day (for instance: the patient may have an exam by one provider, an adjustment by another, physical therapy by a third provider.
We are out-of-network with all insurance carriers, but will be billing for our patients.
While we have had multiple doctors in our practice, I always billed all services for one patient under their specific doctor's NPI. The doctors each did their own exams, adjustments, phys therapy, etc. Now I have patients going from provider to provider on the same day.
I am completely lost on how to bill this and then how to set it up in Medisoft the way I would need it done.
