 Post subject: 1500 Forms: Box 31 & 33

My employer is Nippon Chiropractic & Acupuncture with a designated billing information that we wish to keep in Box 33.

However, when we want to print name the doctor's name in box 31, the Dr. who saw the patient and not Nippon Chiropractic & Acupunture; in order that his printed name can be in box 31 and his signature below it in the same box.

Whenever we print the name of the practice from the "Provider" and "address" page it shows up the same in both boxes 31 and 33. We want to know how to make it so the Practice name is in box 33 and the Doctor's name is in box 31.

Are you filing your claims with Medisoft? If so, you would select to file your claims as a group in your provider IDs grid.

You also may want to look at the Medisoft Help file and in the search field type "Clickable". You can click on each box and it will show you how that box is filled from Medisoft.
