Medicare Secondary Claim

Trying to create medicare secondary paper claim. When I chose a Medicare template I received the following message:
The field practice Id is missing. Please fix the report.
How can I fix it.

Report templates you had setup from prior versions of Medisoft will remain, but you will have to fix them in order to use them. It is usually easier to start with a report template that came with Medisoft and modify that one if necessary. Try using the report template titled CMS - 1500 (Secondary) - 02/12.
