Can't find claim.x12 file.

I haven't done any billing recently because of a broken leg. I tried today and can't find that file anywhere on computer! I'm using v.18 and RevMgmt. I bill to Emdeon & when I search for claim file it doesn't pop up like it used to do. No changes to anything on computer in my absence. Can you help please? Also, I claim I filed in Jan 2014 was rejected because of facility NPI missing. Is this something new? I can't find any info on this subject. I still use codes I set up for clients we see in their homes, an alpha code based on their names. Do I need to do something else here?
