HCFA FORM

I feel like I have had allot of questions latley, but i just had a insurance company tell me that they want all procedure codes on one hcfa form, right now they print seperate if they have a different dx coder associated with them for example

99213-719.44 25 modifier
20610-715.16
J3301-715.16
J2400-715.16
98925-724.2

these all will print on a different HCFA if there are different dx's so can i print all on one hcfa form?

Go ahead and list all the dx codes identically and in the same order on all charge lines. Then click the appropriate dx pointers that goes with each charge line. You may have to delete the claims and recreate them if the charges are already associated with a claim.
